APPENDIX A (SAFETY PLAN)

ACCIDENT PROCUDURE FLOW CHART

• Emergency Phone Numbers:
  From Pool Phone – dial 9 – then # 911
  From Classroom Phone – dial 9– then #911
• Colonie Police Department – 911
• School Nurse SHS – Ext. 3380 or 3378
• North Colonie Ambulance – 911
• Latham Fire District – 911
• Director of Athletics, Ed Dopp, cell 857-8441
• Pool Operator, Mark Wiegert – 857-8426

Rescue, Police and Fire Personnel Access Plan:

1. Park in center oval by canopied walk.
2. Enter the building by left side front door under the canopied walk, (nearest F gymnasium).
3. Proceed down corridor to pool door entrance.

THESE PROCEDURES SHOULD BE GIVEN TO THE RESCUE SQUAD WHEN PHONING RELATED TO AN INJURY.

REPORT PROCEDURE:
Immediately after the accident the lifeguard on duty should file an incident report with:

1. the school.
2. governing organization.
3. Albany County Department of Health (Call 477-4580).

AN INCIDENT REPORT SHOULD BE RECORDED IN THE LOG BOOK IN THE POOL OFFICE.
